目的对2020年广西乡镇卫生院人力资源和收支的现状进行分析,为广西出台加强基层医疗卫生人才队伍建设的政策提供参考依据。方法对广西乡镇卫生院人员数量、编制情况、人员结构、人员流失情况及收支情况进行分析,了解广西乡镇卫生院的卫生人才队伍建设情况。结果2020年广西乡镇卫生院在岗职工共有84 062人,编制人数54 111人,在编人数45 568人,空编率为15.79%。广西乡镇卫生院每千乡村人口卫生技术人员数为3.12人,每千乡村人口执业(助理)医师数为0.95人,每千乡村人口注册护士数为1.07人。2020年广西乡镇卫生院在岗职工以25~34岁者为主(39.85%),卫生技术人员、其他技术管理员、工勤技术人员以35~44岁者为主(41.02%、37.14%、34.21%),管理人员以45~54岁者为主(34.07%)。2020年广西乡镇卫生院在岗职工、卫生技术人员、其他技术管理员、管理人员的学历均以大专学历为主,占比分别为43.16%、45.70%、42.38%、50.16%;工勤技术人员的学历多以高中及以下学历为主,占比58.55%。2020年广西乡镇卫生院共流出人员4 633人,流出比例为5.51%。2020年广西乡镇卫生院收支负盈余的有294家,占乡镇卫生院总数的23.24%;医疗收支负盈余的乡镇卫生院有1 021家,占乡镇卫生院总数的80.71%。结论广西乡镇卫生院编制人数不足和空编率高并存,编制人数与实际需求差距较大;卫生技术人员专业素质偏低、年龄偏高;经济不发达地区的人员流失较严重;盈余少、负债高。这反映出广西基层医疗卫生人才管理体制不顺畅、人员队伍不稳定、优质人力资源短缺及分布不平衡等问题。加快出台系列政策,推进广西基层医疗卫生人才队伍保障机制改革迫在眉睫。
ObjectiveTo analyze the status of human resources and revenues and expenditures of Guangxi rural hospitals in 2020 and to provide a reference for Guangxi for introducing policies to strengthen the construction of a grass-roots medical and health personnel. MethodsThe number of staff, officially budgeted posts, staff structure, loss of staff, and revenues and expenditures in Guangxi rural hospitals were analyzed and the construction of a health talented people team in Guangxi rural hospitals was studied. ResultsThere were 84,062 on-the-job employees, 54,111 officially budgeted posts, 45,568 employees on the officially budgeted posts, and a vacancy rate of the officially budgeted posts of 15.79% in Guangxi rural hospitals in 2020. There were 3.12 health technicians, 0.95 practicing (assistant) doctors and 1.07 registered nurses per thousand rural population in Guangxi rural hospitals. The majority of on-the-job employees were 25-34 years old (39.85%), and the majority of health technicians, other technical administrators, and labor technicians were 35-44 years old (41.02%, 37.14%, 34.21%), administrative staff were mainly 45-54 years old (34.07%) in Guangxi rural hospitals in 2020. Most of the on-the-job employees, health technicians, other technical administrators, and administrative staff graduated from junior colleges(accounting for 43.16%, 45.70%, 42.38%, 50.16%, respectively), and labor technicians mostly graduated from high schools and below(accounting for 58.55%) in Guangxi rural hospitals in 2020. A total of 4 633 employees outflowed from Guangxi rural hospitals in 2020, and the outflow ratio was 5.51%. In 2020, 294 Guangxi rural hospitals were in a negative surplus for the revenues and expenditures, accounting for 23.24%; 1 021 Guangxi rural hospitals were in a negative surplus for the medical revenues and expenditures, accounting for 80.71%. ConclusionIn Guangxi rural hospitals, a shortage of officially budgeted staff and a high vacancy rate of the officially budgeted posts coexist and there is a large gap between the number of officially budgeted posts and its actual demand; the health technicians have low professional quality and older ages; economically underdeveloped areas have serious employee loss; there exist low surpluses and high debts. The situation above-mentioned reflects an unsmooth talented people management system, an unstable workforce, a shortage and an unbalanced distribution of high-quality human resources in Guangxi grass-roots medical and health institutions. It is urgent to speed up the introduction of a series of policies and promote the reform of Guangxi grass-roots medical and health personnel guarantee mechanism is imminent.
