目的探讨宏基因组二代测序(mNGS)在肺部感染性疾病诊断中的应用价值。方法选取2019年8月至2020年5月广西医科大学第二附属医院呼吸与危重症医学科收治的肺部感染患者60例作为研究对象,采集患者肺泡灌洗液标本并进行病原微生物mNGS与常规涂片+培养检测,评估mNGS的临床应用价值。结果纳入研究的60例患者中,进行mNGS检测,检出病原微生物的患者共50例,检出率为83.33%;进行常规涂片+培养检测,检出病原微生物的患者共14例,检出率为23.33%。对肺部感染患者,进行mNGS检测的病原微生物总检出率显著高于进行常规涂片+培养检测,差异有统计学意义(P<0.05)。结论宏基因组二代测序对肺部感染患者的病原微生物检出率显著高于常规涂片及培养检测,应用价值极高,但检测费用较高,临床上可根据具体情况加以应用。
ObjectiveTo explore the application value of metagenomic next-generation sequencing (mNGS) in the diagnosis of pulmonary infectious diseases. MethodsA total of 60 patients with pulmonary infection admitted to the Department of Respiratory and Critical Care Medicine of the Second Affiliated Hospital of Guangxi Medical University from August 2019 to May 2020 were selected as the research subjects. The samples of bronchoalveolar lavage fluid (BALF) in patients were collected to conduct pathogenic microorganisms mNGS and routine smear + culture detection, so as to evaluate the clinical application values of mNGS. ResultsAmong the 60 patients enrolled in the research, 50 patients were expressed in pathogenic microorganisms by mNGS detection, and the positive rate was 83.33%; meanwhile, 14 patients were detected with pathogenic microorganisms by conducting routine smear + culture detection, with the positive rate in 23.33%. For patients with pulmonary infection, the total positive rate of pathogenic microorganisms detected by mNGS was significantly higher than that detected by routine smear + culture detection, and the difference was statistically significant (P<0.05). ConclusionThe pathogenic microorganisms positive rate by mNGS in patients with pulmonary infection is significantly higher than that by routine smear and culture detection. It has a high application value, but the detection cost is also high, and thus it can be applied according to the specific circumstances in clinic.
