振幅整合脑电图在缺氧缺血性脑病患儿诊断及预后评价中的价值

目的探讨振幅整合脑电图在缺氧缺血性脑病患儿诊断及预后评价中的价值。方法选取缺氧缺血性脑病(HIE)患儿45例作为观察对象,根据临床诊断标准分为轻度19例、中度15例、重度11例;选取我院同时期健康足月新生儿11例为对照组。检测分析两组研究对象的振幅整合脑电图(aEGG)特征,在患儿6个月时进行随访,检测比较研究对象的体格发育情况和神经心理发育评分。结果11例对照组新生儿的aEGG检查结果均正常;临床诊断分级为轻、中、重度缺氧缺血性脑病的患儿aEGG检查结果异常率分别为15.8%、86.7%、100.0%,缺氧缺血性脑病患儿的aEGG检查结果与其临床严重程度密切相关(r=0.632)。6个月时随访发现,aEGG重度异常组患儿的头围、身长、体重及智力指数(MI)、发育商(DQ)结果均差于其他三组研究对象,其中头围、MI、DQ组间比较,差异有统计学意义(P<0.05);身长、体重组间比较,差异无统计学意义(P>0.05)。结论缺氧缺血性脑病患儿aEGG检查结果的异常程度与其临床分度密切相关,aEGG可以用于评价患儿的脑功能状态以及对患儿进行早期诊断、判断病情及预后。
ObjectiveTo investigate the early diagnostic and the prognostic value of amplitude integrated electroencephalography (aEEG) in neonates with hypoxic-ischemic encephalopathy (HIE). MethodsA total of 45 patients with hypoxic-ischemic encephalopathy(HIE) were selected and divided into mild HIE group (19 cases) , moderate HIE group (15 cases) and severe HIE group (11 cases) according to clinical diagnostic criteria, and 11 cases of healthy full-term newborns in our hospital at the same period were chosen as the control group. The aEGG features of all cases were observed, physical development situation and neuropsychological development scores were detected after 6 moths. ResultsThe aEGG of the control group were normal; the abnormal rates of aEGG in mild, moderate and severe hypoxic-ischemic encephalopathy group were 15.8%, 86.7% and 100.0%,respectively, and there was positive correlation between aEEG and HIE clinical indexing (r=0.632). After 6-month follow-up, head circumference, height, weight and MI, DQ in severe group were worse than those in the other three groups and there was significant difference in head circumference, MI, DQ among four groups(P<0.05);while the differences of height, weight among four groups was not significant (P>0.05). ConclusionsThe abnormality of aEGG is closely related to HIE clinical grading, and it can be used to evaluate the status of the brain function, and can assist in early diagnosis and predict prognosis of HIE.
