Objective To investigate the effect of a WeChat‑based "three‑professional co‑management" model on blood pressure in middle‑aged individuals at high risk for stroke. Methods A randomized controlled study design was adopted. A total of 111 middle‑aged individuals at high risk for stroke, screened at a community health service center in Nanning, Guangxi from January to October 2024, were enrolled as research subjects and assigned to a control group (n=56) or an observation group (n=55) with a random number table method. The control group received routine offline community health management delivered by physicians and nurses from the community health service center; on this basis, the observation group additionally received an intervention of WeChat‑based "three‑professional co‑management" model (involving tertiary hospital medical specialists, community health service center general practitioners, and brain‑heart health managers). The two groups were compared in terms of stroke knowledge questionnaire scores at the 6th month of intervention, as well as systolic and diastolic blood pressure before intervention, at the 6th month of intervention, and at the 12th month of intervention. Results Within each group, there were statistically significant overall differences in both systolic and diastolic blood pressure across the different time points (all P<0.05); pairwise comparison results showed that the systolic blood pressure and diastolic blood pressure of the two groups decreased gradually before intervention, at the 6th month of intervention, and at the 12th month of intervention (all P<0.05). Intergroup comparison results showed that there were no statistically significant differences in diastolic blood pressure, systolic blood pressure, and the total score of the stroke knowledge questionnaire between the two groups before intervention (all P>0.05); at the 6th month of intervention, the total score of the stroke knowledge questionnaire in the observation group was higher than that in the control group (P<0.05); at the 6th and 12th months of intervention, the systolic blood pressure and diastolic blood pressure in the observation group were lower than those in the control group (all P<0.05). Conclusion The WeChat‑based "three‑professional co‑management" model can effectively control blood pressure and improve disease awareness in middle‑aged individuals at high risk for stroke.
