目的探讨在常规治疗基础上加用硝酸甘油及地尔硫卓治疗慢性肺源性心脏病急性加重期患者的临床价值。方法选取120例慢性肺源性心脏病急性加重期患者随机分成对照组和观察组各60例，对照组患者采用控制感染，畅通呼吸道，改善呼吸功能，纠正缺氧和二氧化碳潴留，控制呼吸和心力衰竭，积极处理并发症等措施治疗。观察组在上述治疗基础上，加用硝酸甘油注射液同时联用地尔硫卓进行治疗。比较两组患者临床症状（气急、乏力、心慌等）和体征的改善情况，以及治疗前后左心室和右心室射血分数、心率和平均住院天数。结果两组患者临床症状及体征均有改善，但观察组的改善程度明显优于对照组；观察组右心功能的改善和心率的控制也优于对照组；观察组住院天数少于对照组。结论对慢性肺心病急性加重期患者，常规治疗基础上加用硝酸甘油及地尔硫卓进行治疗是有益的。

ObjectiveTo investigate the clinical value of chronic pulmonary heart disease for patients with acute exacerbation in the general treatment based on the use of nitroglycerin and diltiazem.Methods120 cases with chronic pulmonary heart disease at acute exacerbation stage were randomly divided into observation group and control group with 60 cases in each group.Patients in the control group were used infection control,unobstructed respiratory tract,improving respiratory function,correcting hypoxemia and carbon dioxide retention,control of breathing and heart failure,active treatment of complications etc.The observation group combined with nitroglycerin injection and diltiazem treatment.Comparative observation of two groups of patients with clinical symptoms (fatigue,shortness of breath, palpitation etc.) and symptoms improvement before and after treatment,and the left ventricular and right ventricular ejection fraction,heart rate and the average length of stay.ResultsAlthough the results of two groups,clinical symptoms and signs were improved,but the treatment group is better than the control group.The improvement in right heart function and heart rate of the treatment group was significantly effective.ConclusionNitroglycerin with diltiazem is of benefit to chronic pulmonary heart disease in patients with acute exacerbation.
